Byddai’r plant yn gwerthfawrogi gymaint o ymatebion a phosib, felly os oes gennych chi 5 munud dros y penwythnos, tybed os allwch chi lenwi’r holiadur isod? Gall ddisgyblion, rhieni, teuluoedd a ffrindiau gwblhau’r holiadur ac mae’r plant wedi sicrhau ei fod yn ddwyieithog.

As part of their Science work, Year 5 have created a questionnaire about healthy living. The children would appreciate as many responses as possible, therefore if you could give 5 minutes of your time to complete the questionnaire below, it would be much appreciated.
